Council Dashboard forms allow you to generate leads for the community that can be shared automatically or manually with linked groups.
If you want to customize your Council Dashboard form to meet your needs, follow these steps:
Navigate to the “Forms” tab by clicking on the “Forms” button on the left-side navigation bar.
Create a new form or click the “Edit” button on an existing form.
Enabling Features - any of the features below can be enabled by toggling them on, on the right side option box. See screenshot below.
Images - do you want those completing your form to be able to share an image? If so, enable this.
Referrer Information - do you want someone to be able to referrer someone through this form and collect that referrers information too? If so, enable this.
Confirmation email - you must type in your content for the option to be available to enable. Note, this email will be sent upon successful completion of your form.
Collect payments - do you want to charge a fee on your form? If so, enable this. Note there are more steps you will need to take. Learn more here.
Agreement - do you have a grade release or terms and conditions you want someone to review and agree to? If so, enable this.

4. Add custom fields - custom fields are where you ask the specific questions to collect the information that is most important for you and your community. To add these navigate to the “Form Fields” section and click the “+Add Field” button in the right corner. This will allow you to create custom fields.

5. Create your confirmation and agreement (if applicable)
You enabled your confirmation and agreement, now you need to input your content. Simply copy and paste or type in what you want to say. You can always send a test confirmation email. You can also view your agreement by clicking the “View Form” button in the bottom right corner.
SAVE CHANGES - As you make changes to your form, you will see a pop-up save button at the bottom of your screen. We encourage you to click that as it appears to ensure your work is saved.
