ChapterBuilder - Form won't load for people to complete

Are individuals trying to complete your ChapterBuilder forms having a hard time getting it to load? Is the form just flashing like the image below?

This means you have a broken link for your form. This means you probably did one of two things or potentially both:

Incorrect link - make sure you are getting the correct shareable link. You CANNOT copy the url, you must click on the "Copy Form Link" to get a shareable link.

Shortened your link - the service you used broken the link when shortening it. We recommend you use a different service.
